Case Management / Care Coordination Program
Case management strives to assist clients and families to navigate through healthcare systems. Case managers provide advocacy for clients and interact with the members of the interprofessional healthcare team to find solutions in providing effective, quality, and efficient care. Case managers provide care coordination to communicate the needs of the clients and guide the delivery of appropriate and effective care.
The program was designed utilizing the Standards of Practice for Case Management as endorsed by the Case Management Society of America.

Who Should Enroll in this Program?
This program is designed for English-speaking nurses and other healthcare professionals who want to enhance or develop the knowledge and skills necessary for quality case management.
Learning Outcome
The learning outcome of this program is to enable the participant to successfully implement care management (care coordination) practices, ethics, and standards in their clinical settings through interprofessional collaborative practice.

Program Information
Objectives
At the conclusion of this program, the participant will be able to:
- Present an overview of the case management process.
- Identify methods to coordinate services to meet client needs.
- Explain the financial aspects of case management.
- Integrate disease management and case management into the plan of care.
- Apply case management principles to clinical situations.
- Assimilate evidence based interprofessional collaborative practice.
Criteria for Successful Completion
- Submit registration and payment.
- A certificate for contact hours will be provided following the completion of each module study guide, receiving a passing score of 70% or better on the comprehensive final exam and submission of the program evaluation.
The certificate of completion will be sent via email immediately upon the completion of the program evaluation.
Successful completion of this program does not guarantee expertise in the content. Clinical experience and judgment, coupled with an understanding of the content are necessary to attain competency. Achieving certification via a designated professional organization is an additional step to demonstrate competency.

The University of Southern Indiana Center for Health Professions Lifelong Learning designates this activity for 40 contact hours.
Enrollment Cost - $660
The fee includes access to the program along with all resources and handouts. A text, if applicable, is not included in the fee.
Registration by invoicing employer closes two weeks prior to the start date. There is a $25 fee to invoice.
The deadline for registration is the Wednesday prior to the start date or until program reaches maximum capacity.
We reserve the right to cancel any offering if there is insufficient enrollment. Registered participants will be notified in advance and fully refunded if this occurs.
Registration may not be available until closer to the start date.
Refund Policy
A $75 administrative fee will be charged prior to the start date of the activity if the registration fee has been paid. On or after the start date of the class there will be no refund.

Program Content Delivery
Program Content
The content is divided into modules. Each module utilizes instructor outlines, audio/video recordings, reading assignments, articles, web resources, case studies to augment the learning process. These methods provide the participant with flexibility in completing assignments and interacting with the faculty. Each module has accompanying study guide(s) with questions (such as multiple choice and essay) for participants to submit to the instructor. Interaction with the instructor may occur via study guides, discussion boards, and or email. The modules cover case management principles, case management process, care coordination of services, financial aspects of case management, disease management, and application of care management.
A textbook is required for this program. Upon registration, you will receive detailed information concerning the textbook.
Delivery of Content
The content is delivered asynchronously via the online learning management system, Blackboard. The internet provides an opportunity for interaction with program faculty while in the comfort of learning in your own home and office. Program staff are available during business hours by phone or email to assist participants with technical issues or concerns.

This activity was planned by and for the healthcare team and learners will receive 40 Interprofessional Continuing Education (IPCE) credits for learning and change.
In support of improving patient care, this activity has been planned and implemented by the University of Southern Indiana Center for Health Professions Lifelong Learning (CHPLL). The CHPLL is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), and the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program to provide continuing education for the healthcare team.
This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers. This course is approved for 40 CE Contact hours.
