How Do I Apply to CAP?

Acceptance to CAP requires that a student meet program and course prerequisites. Follow the steps below to get enrolled!

a. Review the courses available at your high school

c. If interested in attending USI, request to meet with a USI advisor

Request Your CAP Courses

Submit Online CAP Application

First, you will need to create an application account (if you have not applied to USI before).
To complete your Online CAP Application, you must have a parent/guardian email address that is different from your email address.

Your parent/guardian will be sent a link to an electronic Student Permission Form after you submit an Online CAP Application.
The Parent/Guardian Permission Form is also available through a PDF, linked below if needed. 

Request Your CAP Courses

Your parent/guardian will be sent a link to an electronic Student Permission Form after you submit an Intent to Reenroll.
The Parent/Guardian Permission Form is also available through a PDF, linked below if needed. 

My Parent/Guardian Does Not Have An Email

If you have access to a printer, print the Student Permission Form; take it home for a signature and return it to your CAP instructor.

If you do not have access to a printer, see your high school guidance counselor for a paper Student Permission Form. Take it home for a signature and return it to your CAP instructor.

I Am A Ward Of The Court or An Emancipated Minor

If you are an emancipated minor or a ward of the court, your counselor should sign the CAP Student Permission Form.
