COVID-19 Vaccination

USI Vaccination Clinic for the General Public

To make an appointment, visit the State of Indiana Vaccination appointment portal and choose USI as the location. 

What do I need to bring to my appointment?

  • Your Vaccination Card if returning for a second dose
  • A valid driver's license or other photo ID
  • It is a good idea to eat and drink something before your appointment.

Clinic Dates

Every Tuesday and Thursday that the University is open through Tuesday, December 21.

Where do I go for my appointment?

The USI Vaccination Clinic is in the University Health Center, located on the lower level of the Health Professions Center, room HP0091. Enter at the southwest corner of the building. There are signs to help guide you for parking and clinic location.

What brand of COVID vaccine will I receive?

The Vaccination Clinic is currently vaccinating patients with the FDA-approved Pfizer vaccine. This vaccine requires two doses for people age 12 and older to be fully vaccinated. Children under 18 need to have parent consent.

After vaccination, you will sit for a 15-minute observation period to watch for rare allergic reactions.

How much will the COVID vaccine cost?

COVID vaccine doses administered by the USI Vaccination Clinic will be administered at no cost. Insurance information is not required.
