COVID-19 Vaccine Clinic Registration Information 

Please read! Registration is required to be vaccinated.

On Monday, April 5, you should have received an email from the State of Indiana to register for USI’s COVID-19 Vaccination Clinic to be held Wednesday through Friday, April 7-9, in the Screaming Eagles Arena. NOTE: the Clinic will close at 5 p.m. on Friday, April 9.

The email you received should look similar to the example below, and came from

The Indiana Department of Health invites you to receive the free COVID-19 vaccine at Zotec Site Name. Please use the link below to make an appointment. This link is unique to you. You will schedule any necessary second doses when you receive the first dose. Call your campus if you need assistance scheduling your appointment.

If you did not receive your email on Monday, or if you are having trouble setting up an account and registering, please contact the University Health Center at 812-465-1250. Additional information about USI’s Vaccination Clinic can be found on the USI website.
