Request for Alternative Work Arrangements


As a result of the COVID-19 pandemic, most employees have been away from the USI campus since March 17. We are approaching the time for a safe return to campus beginning July 6 (based on guidance available today). The Human Resources (HR) process for submitting a request for alternative work arrangements or considerations is now available for the period between July 6 and the end of the Fall semester, December 22.

The University is committed to ensuring that your submission remains private. This option is strictly voluntary. The information you provide will be reviewed by HR and only approved HR employees will have access to your medical information. You will be contacted by HR or your supervisor regarding the alternative work arrangements or considerations you are requesting.
Who is eligible?
Older employees [defined as age 65 and older by the Centers for Disease Control and Prevention (CDC)] and employees of any age who have serious underlying medical conditions might be at higher risk for severe illness from COVID-19, and may contact HR to request alternative work arrangements or considerations. HR may also be contacted to submit a request for alternative work arrangements or considerations, if an employee has a household family member in any of the high-risk categories outlined by the CDC.
These include:
• 65 years of age or older
• Chronic lung disease or moderate to severe asthma
• Serious heart condition
• Severe obesity or a body mass index (BMI) of 40 or higher
• Diabetes
• Chronic kidney disease and undergoing dialysis
• Liver disease
• Condition which causes me or a household family member to be immunocompromised
