For any event involving children, the Program Administrator (PA) must fill out the Event Planning Form for Programs Involving Children.
Event Planning Form for Programs Involving Children is submitted to the Child Protection Committee (CPC) for review.
CPC determines whether background checks are required for all individuals or a limited number of individuals assisting with the event.
PA must have participants fill out both Program Participant Form and Authorization for Background Check and Sex Offender Registry Checks Form.
|PA completes background checks as determined by CPC and sex offender registry checks for all community and student volunteers assisting with the event.
PA Request to HR
|PA must request Human Resources complete background checks and sex offender registry checks for all University Employees assisting with the event.
|HR sends PA outcomes
Participants complete Child Protection Policy training (prior to the event). Online training can be found at myUSI. After logging in, click the Online Training icon that looks like a yellow certificate found under Eagle Apps on the left side of your myUSI homepage. Then click the CPP icon that appears on the USI Training page.
PA records dates of background check and/or sex offender registry checks on Background Check and Sex Offender Registry Tracking Form.
PA or background-checked designee completes Program Participant Check-In Form.
Participants who have not had training (as indicated above) must read and sign a copy of the Volunteer Child Protection Policy Training.
Note: Program Administrators must familiarize themselves with the Child Protection Policy, the Child Protection Policy FAQ’s, and complete the University-approved training of this policy. Program Administrators must keep all student and volunteer participant forms and records. HR must keep all employee background and sex offender registry checks.
