Mary McLaughlin Davis, DNP, MSN, APRN, ACNS-BC, NEA-BC, CCM, President of the Case Management Society of America and Senior Director of Care Management with the Cleveland Clinic Health System, will provide the keynote address.  The conference will focus on the role of care coordinators, case managers and navigators through all phases of care delivery.  Attendees will be able to implement effective care coordination strategies for a variety of clients.

In addition to a keynote and concurrent presented by Dr. McLaughlin Davis, a variety of topics will be covered including State Health Insurance Assistance program (SHIP), Adult Protective Services, Hospice/Palliative Care, Veterans Care and Ethics.  The day will conclude with stress relief strategies presented by nationally recognized expert and humorist Bobbe White.  This conference is appropriate for Case Managers, Care Coordinators, Navigators, Discharge Planners, Nurses, Social Workers, Advanced Practice Nurses, Physicians, Students and any healthcare provider concerned about coordination of care.

Registration fee: $105, $50 for graduate students, and $15 for undergraduate students until April 25; thereafter, $125, $70 for graduate students, and $25 for undergraduate students. Includes lunch & refreshments
